Sports and activity form templates

Run clubs, leagues, tournaments and team seasons: sign-ups that fill, close and tell you who plays.

Picking a form for your team or club

Start from how people join. A pickleball league or a padel tournament fills by level or category, and the form closes when every place is taken. A run club takes everyone and sorts them by pace.

For kids' teams, a parent registers each child once at the start of the season with the youth sports registration, then answers a game availability form each week.

Outdoor groups like hikes and rides ask for an emergency contact. Races, camps and tournaments add a plain risk acknowledgement or a guardian for under-18s; swap in your insurer's wording if they give you one.

Sports form questions

Can players pay the entry fee in the form?

No, Fillio doesn't take payments. Put the price and a payment link in the intro, or collect at check-in. Each reply shows who signed up, so you can tick off who paid. Places on a limited form are taken when someone signs up, not when they pay, so ask for payment within a few days and delete replies that never pay to open the place again.

Can one person register a whole team?

Yes. The 3x3 basketball tournament and golf scramble forms ask the captain how many players there are, then ask for each player's name and details once per player. Other sports forms take one person per reply, which keeps sizes, levels and emergency contacts tied to each player. To take whole teams on another form, add a number question for team size and repeat the player questions once per player.
