Baptism RSVP
Guests confirm for the church and the lunch, so both counts are right.
What's inside
Families who come tick the ceremony, the lunch or both and give a headcount with the number of children. Families who can't come skip those questions and get a warm goodbye; everyone can leave a blessing.

Planning the baptism day
Send the link three to four weeks before. Put the church, the time and where the meal is in the intro, since some guests come only to one of them.
Counting the ceremony and the lunch separately matters more than it looks. Colleagues and neighbors often come to the church only, while family stays for lunch, and the restaurant or caterer needs the second number.
The number of children helps with seating and a kids' menu. If you're asking the godparents to stand with you, tell them in person before the invitations go out; the form is for everyone else.
Print the blessings and keep them with the baptismal candle or the certificate. Later years bring the first communion, which works the same way.
Baptism RSVP questions
Can I use it for a christening or a dedication?
Yes. Change the title and the intro; the questions fit a christening, a baby dedication or a naming ceremony equally well. Rename the ceremony option to match, such as the dedication at your chapel or a naming at home. For an adult baptism, rename the children question so it counts the kids coming along, or delete it.
Can I write it in Spanish or Portuguese?
Yes. Rewrite the questions, options and end messages in your language once you create the form; the logic follows the answers, not their words. For a family with two languages, write each question in both, one after the other, so grandparents and friends read the same form. Keep each line short so the pair still fits on a phone screen.
What if there's no meal after?
Delete the ceremony-or-lunch question. The headcount and the blessing still work on their own. If you serve cake and coffee after the service instead of a meal, keep the question and rename the lunch option, since even a small reception needs a rough count. Change the intro too, so guests know what follows the church.
